To all persons, be it known that our company, as grantor, do hereby make and grant a limited and specific power of attorney to members of the staff of Arizona Contractor License Center, Inc. and Utah Contractor License Center, Inc.
My named attorney in fact shall have full power and authority to undertake commit, and perform only the following acts on my behalf to the same extent as if I had done so personally; all with full power of substitution and revocation in the presence.
To register, file and/or obtain any or all of the following, including certified copies or originals of:
- Business Name and/or Sole Proprietor Registration, Articles of Organization or Articles of Incorporation.
- Amendments to Doing Business Name (DBA) Registrations, Articles of Organization, Articles of Incorporation.
- Liability Insurance to include certificates of insurance.
- Worker's Compensation Insurance to include certificates of insurance.
- Verification of license(s) from State Licensing Agencies.
- Statutory/Registered Agent services and annual renewal filings.
- Dissolution of Limited Liability Company, Corporations and releasing business names
- Any correspondence with State Licensing Agencies
The authority granted shall include such incidental acts as are reasonably required or necessary to carry out and perform the specific authorities and duties stated or contemplated herein.
Grantor assumes the risk of all damages, loss, costs and expenses, and agrees to indemnify and hold harmless Arizona Contractor License Center and Utah Contractor License Center its officers, agents, and members of staff for any claim, suit or action brought against Arizona Contractor License Center and Utah Contractor License Center for the preparation, assistance, consulting, and filing of applications and registrations to obtain a contractor's license.
